[1] An example of the user registration is sent to all participants by e-mail.
[2] After you submit the registration form via web site, you will get user ID and the password from KEK users office about 1 day later, then, you will be able to submit “Visit Proposal” from your user page.
[3] If you have already had the user ID, please add “Identify the purpose of your visit to KEK/J-PARC” from your user page. For detail, please contact the school office.
[4] Please send the form in scanned file by e-mail attachment. The signatures of your supervisor and your affiliation’s radiation control officer should be on the form. And also you are required to submit the original one to the school registration desk on December 1, the first day of the school.
[5] We will accept credit card and in cash in Japanese Yen, but credit card payment is strongly recommended.
[6] The form is sent to all participants by e-mail.
